Reminder to families all medication that is needing to be administered at school or on an excursion, must have the above form filled in by the doctor or pharmacist along with all medication having a pharmacy label stating the childs name and instructions on dose and frequency of medication.

This also applies to all medication that hasn't been prescribed from the doctor, for example travel sick tablets, panadol, anthisitmaines.
